Not working on my Amazon prime signature Chase card with no outstanding balance. This offer makes no sense how it works on some accounts and not others.
Activated on my Amazon card on the 2nd. Did a few small purchases to test out the 5% back at grocery stores (Safeway & Aldi), but I'm still seeing 1% back on Chase's website.
Fine print says 'Please allow up to 8 weeks after your qualifying purchases to post to your account' which is odd, bc a few Chase Bonuses I've activated in the past were pretty quick to show the bonus points.
I guess I'll use my Discover IT until I confirm the 5% is indeed posting for the Chase Amazon card.

The full 5% back is showing up for a pretty big purchase I made at Publix on July 3 with my Prime card. It's broken down as "Bonus on purchases at grocery stores," which shows 4% in points, and "1% Back on all other purchases,"which shows that amount in points for a total of 5%.

Darn. Still nothing here.
I remember my freedom unlimited had the same bonus category a few quarter's back, and the bonus point details were exactly how you described.
Going to try the AmazonPrime card on a restaurant too since that was one of the bonus categories.
When using the Chase cards make sure to check for Chase Offers. For example, right now on my Chase Amazon Prime card I have 2-5% offers from Chevron, RaceTrac and Circle K. When activated they stack on top of the 5% gas
I tried this. I have the Circle K coupon clicked/enabled. I also have this promotion from OP enabled. I spent $48.16 at the pump.

It appears to cap out at 5% max. It won't stack to 10%.
Also, it appears there is no advantage to buying at Circle K/Chevron with both promotions enabled.
I wonder if the 2% ($0.96) counts towards the $1000 limit on this promotion since I would get it anyways ("non-Bonus")? The website doesn't show a progress meter towards the promotion limit like Discover has for their quarterly promotions.
Does this stack with the 2% you get back normally at restaurants?
Based on what I just posted for my Circle K gas pump transaction, I'm going to say "no". You're capped at a 5% discount. Still unsure what portion of that counts towards the $1000 limit.
I'm waiting for a pending Wendy's counter transaction to clear from Wednesday. It's categorized as "food and drink", not "restaurant/dining". I hope it won't get screwed up.

Merchant cash back is separate from direct cash back from Chase, and can take longer to post. They don't show up lumped together.
I have had the stacking work on Freedom cards on top of the 5% quarterly cash back. Perhaps it is different on the Amazon Prime card with this special bonus. But I see no indication of that in the fine print.
Chase has a Rewards box at top right of the website. There is a clickable arrow to Amazon Rewards Points where things are broken down by category and percentages.

I'll see if a merchant cash back amount comes later. How long would you expect it to take? I hope you're right, I pass Circle K on the way home every day. They usually charge 10 cents over the cash price to use credit, but 10% back would be like 36 cents per gallon or so.
I know Chase has a category breakout but it's still unclear if only 3% of the fuel purchase counts toward the $1000 limit. Just the "Bonus", or all 5%? Discover has a nice progress bar showing how much more spending you can do within a category during their promotion towards the quarterly $1500. Once I hit the $1000 i'll go back to using my BoA card with 3% on gas and my WF card that pays 2% no categories.
My Wendy's purchase did yield 5%, too, so that's good.
