It's interesting to me that they say "drug free" but melatonin is a hormone and it's considered a drug in many other countries. I also consider 3mg a fairly large dose. Another interesting thing about melatonin is that many people even find effectiveness at .5-1mg range.

Alcohol is the worst thing for sleep. It inhibits REM sleep. Why do people think red wine is some magic juice. It's the same alcohol as any other alcoholic drink

That's a very fair assessment. Insomnia won't be cured by most drugs, even melatonin.

I had reconstructive surgery on my shoulder and the last 2 months my spouse has helped so much! Had no idea, has to sleep in another room, as I haven't slept much. Finally, I thought to take a Natrol 5 mg melatonin last night. Worked too well. Overslept this morning, but I will try 3 mg Natrol next.

Time to find a different doctor I guess or a sleep specialist. Some cardiologists and pulmonologists have good experience with sleep issues. There's also a host of virtual providers now even through Amazon but not sure how good they are. I tried One Medical a few times and most of them seemed decent and younger doctors so more willing to listen. Hope you find something to help and eventually you should of course aim to not take anything or not often at least. CBT-I and things like yoga nidra work well for many people, but requires time and diligence. Perhaps look into that if you continue to have issues when you have time.

There are websites and apps out there that will allow you to search for doctors that take your insurance, practices in your area and that cover the specialty or general practice that you need. Apps like ZocDoc, or your provider might have its own app that will help you look for a new doctor. Please look for a new doctor, you deserve better care.
Also maybe consider getting a fitness watch like Fitbit or Apple watch. While these are designed for activity many of them also monitor sleep, they can give you an idea of what your sleep really looks like and sometimes doctors just need to have a visual of what you're talking about as opposed to saying I don't sleep well. Something that helped my sleep tremendously that I haven't seen here yet: night time eye mask.

Minimal screen time before bed helps, but the mask helps keep things dark and that has done more for my sleep than anything else so far (up there with streaming rain sounds or other sleep noise stuff).
