Due to repeated questions in the thread here are additional details.
The $300 Verizon GC offer is tied to purchasing a smartphone online. You are eligible to claim this offer for every order transaction regardless if you are placing an order for 1 phone/line or 10 phones/lines.
The $200 Verizon GC offer folks have been receiving via sales chat/phone is a switch promotion. I am not able to find the terms for this offer but in the past this has been an available offer which requires you to port in from a non Verizon carrier that includes the various Verizon prepaids and MNVOs.
To my knowledge these Verizon GC offers have 12 month service terms or you will be charged back "claw back"
There are currently 2 separate iPhone 16 offers.

When you add the phone to the cart you must choose the right offer otherwise you will not go through the workflow with the step to add a trade in.
In addition if you enter a non Apple, Google, or Samsung device during the trade in you will receive the actual value of the device, including $0 if its older to be recycled,
AND you'll be defaulted to the non trade in promotion.
This is the reason at checkout some people are seeing $10/month or $5/month because they've selected the Welcome or Plus plan respectively and the non trade in offer has been applied.
There have also been a lot of "can I trade in XXX phone?".
The terms state you can trade in ANY Apple, Google, or Samsung phone in any condition. In my experience the any condition is intentionally meant to covered cracked screens and aesthetic damages. The device should turn on and function well enough that the IMEI can be retrieved and have no swelling/damaged battery (for safe handling reasons). Regarding any phone from the 3 manufacturers the process is constrained by the system and the list of devices/imei in its database. Be reasonable and don't expect extremely obscure or foreign region only devices to come up.
You can determine for yourself if a device qualifies for the $1000 by going through the online order process. You will need to enter the IMEI so you must have the device in hand. If you are not getting the $1000 value then you likely have not selected the right offer when adding the phone to the cart or your device is too obscure. I've attached screenshots of 2 devices that have been inquired about (A03 and 5c) showing the $1000 trade in amount.

Anyone have idea what happened here? This was for a store pickup order. The order amounts changed between the process and I didn't realize that - what a cluster!

I am also ready to mail two moto phones for this deal. I am just a bit worried because the fine print only says eligible trade-ins are Google, Apple and Samsung. But when the sales agent placed the order, apparently a bunch of moto phones have trade-in values of $1000 as well.

Now she is offering one time $200 on top of 4 $100 gift cards (one per line for 4 lines I was planning on transferring). Should I just keep trying with another rep.

second rep was great and worked with me all the way through almost completing the order, but chat got interrupted and I had to start all over again with rep 3. Rep 2 insisted I could only get $100 per line, but offered to sweeten the deal with one time $200 added to the transaction, which would have been $600 credit. Because of disconnect, I was forced to continue with rep 3.
third rep was also good, except was able to apply $200 offer to each line. I was able to complete the order successfully.
To contribute to this great deal, here are few tidbits from my dealing with Verizon.
1. I started chat with "I saw you have a new promotion - "iPhone 16 Pro up to four, on us" deal. I would be potentially interested in porting 4 lines to create a new Verizon account. Specific details I am interested in: Unlimited welcome plan, $200 port in rebate per line (promotion code CHAT200GC2024) and waiver of activation fees. Is this something you can help me with please?"
2. you will need to fill out soft credit report, the rep will provide the link, using Chrome, I had no way to open the link in another window. Once you get the link, click it, fill out all the info, submit. Make sure you get submit successfully message. After that click back button in the browser, and hopefully you will end up in the same chat window wish same representative. I was using a PC, maybe its different on other devices, but definitely sketchy experience.
3. have imei numbers ready, as I was doing 4 phones, I had 4 numbers. They rejected 2, not quite sure why, and they were unable to elaborate. Seemed kind of random, one was iphone 7plus and another iphone 5s. When I asked for which devices qualified, she said anything down to iphone 3g. I traded in 2 iphone 6s, iphone 7 and iphone x, all for $1000 credit.
4. when you finally get to completing order, they will provide another link in the chat, ask them to send it to you via sms, this way you are not compromising chat and able to see the deal on your phone, while still chatting with rep. When I opened the link, the $200 rebate was not listed for each device, but was listed at the top of the order as "you are eligible for $200 verizon gift card on us". I started questioning the rep, because I expected to see it on each device. She was trying to reassure me that it was for each device, but visually, it was not that way on offer. Actually I would say less risk than going through chat agent - I have screen shots of all orders showing the gift cards included. I have already submitted 3 of the rebates ($300+200+200)
Extra Cashback makes it worth the risk for me.
The only thing I'm worried about now is the first bill, I was prepared for it being higher than expected, but they have my first bill estimate at $480 . Second bill shows correct at $150 w/ tax. Hopefully they are wrong on the 480. I guess I have to call and try to get the activation fees waived.

If you don't prefer that. You can do $150 instant credit through promo code per device (works on 16PM, not 16P) which brings the device cost down from $5.55/mo to $1.38 per month + $300 gc. Total $900 off.
I honestly would've preferred doing the instant credit - that way I wouldn't have to wait on GC and possibly be denied. But didn't know the $150 promo code was really 1 time use.
Edit: adding DP. Can't get the 200 GC promo to show up for my 3rd transaction. May be limited to only 2 GC.
