EDIT: I guess Metro doesn't have much of a time limit between when letting your account expire and placing new orders because I just placed an order for this ip13 and it's only been about 3-4 months since I let my previous metro plan expire. Also, I placed the order just minutes after creating a visible account and porting that new visible number over to metro.

I don't think you can, once you port out your first line I believe visible closes your account automatically so you would need to create a new account and go through the referral route to get another $5 line
yesterday. Today I got the activate email where you
input your order#, last name, and zip to activate. After doing that the "Activate"' button is grayed out. ! called
Metro and agent said to wait until it shows delivered on
their site (since it still shows delivery on 2/24, but UPS
delivered on 2/21) and try again. Is this an issue that
anyone else has faced before and how did you resolve it?

I would wonder why, for #4, they'd allow numerous orders via that particular department if their web site states that the limit on this deal is 2 lines.

I just took my first and upgraded it to an S25 on Spectrum for $17, and my MKSD arrived right after I gave it up. I'll test on the new 13 when it arrives Monday.
