75% of the lines in those movies are ad-libbed. They have a general set of lines for direction, but they don't stick strictly to a script. They just do a lot of takes and use the one they like.

Medical expenses are the costs of diagnosis, cure, blah blah this or that blah blah yada yada lots of words.
Medical care expenses must be primarily to alleviate or prevent a physical or mental defect or illness. They do not include expenses that are merely beneficial to general health, such as vitamins or a vacation.
