Does that mean it should be OK to wait longer? I've heard that some people have had no trouble activating these 3-month prepaid cards from Best Buy after longer than 45 days, but not sure if that is common or if its the exception...

The value of the freebie is $60, which is greater than the price of the phone, so it sounds like if they stick to that policy you could return the phone, but you'd get no refund at all for it.
