1) Signed up on AT&T website and populated with CC info and requested to port my number. Received card in 3 days and charged only for SIM (4.99 + tax).
2) Tried to activate the sim about 10 days after receiving it and had numerous issues. The AT&T LTE network was connected with the new SIM, but it wasn't working. I tried activating through the website, automated phone system (called 800-331-0500). Could not get a live person. SIM card comes back as active but on another account. I could not access my prepaid online account. I couldn't even do a temp password reset or PIN reset. I couldn't call 611 (or any number) from my mobile phone as the line wasn't working.
3) Went to an official AT&T store and person tried the various methods to activate, and could not. He said my SIM shows as active and the port is "confirmed". Everything looked good, but he had no idea why the port wasn't completed. So he had to call IVR (*243? or something like that from his phone). It's an internal AT&T support number, not the same as the 800 number I used. He spoke to an agent who saw the same thing. She said the port needed to be manually completed as there was a "future date" for activation. That worked...sort of. I could now call.
4) I could now call 611 from the phone and access my prepaid account online. I still had no data and could not call any other numbers. The operator said I needed to prepay my $300. I paid with my credit card and was charged $300 + taxes, total came out to a little over $310. I realized I could've save a bit with gift cards/etc, but oh well.
5) I still couldn't make phone calls or access data for about 5 minutes. So I did a restart, and the phone worked after. I made a test phone call and it said "welcome to AT&T prepaid" and then dialed. I could also access data.
I have no idea why the process needs to be so complicated. AT&T needs to get their act together, it shouldn't be a difficult process.
Also, I click on one of the link you put. It got me to the log in page where I have to put in my phone number and password to log in. Say, if the agent on the phone helps me port the number over successfully, will they give me a temp password to log in?
I guess after I log in I can load those refill card (email delivery) and use it to pay.

Mine said it was activated but I couldn't call 611 for customer service on the device and I couldn't log into the account post activation screen. Had to wait till wife came home with her phone to call support. Credit card only showed a charge for $5 sim at this time.

Mine now says "completed" but before and during activation it stated something different I believe. I entered the number to port during original checkout and I checked my old carriers website and it showed the number had been deactivated and service canceled yesterday. ATT Chat told me service / SIM was activated and to wait 45 minutes, power cycle phone, and all should be good. Still couldn't call 611 or any numbers on device an hour later. When I did finally get CSR on wifey's phone I told CSR I had activated the line on the SIM/IMCI with the link above and it was successful, I was porting a number, old carrier showed line was deactivated / disconnected, and then relayed to CSR whatever the "port status" link showed above. She put me on hold, told me the port was still processing but she would push it, hold again, and she returned saying the port was done and I'd need to pay the $300 (plus taxes) to get service activated. Gave her CC info and shortly afterwards did a test call with my phone and verified data was operational.
Ironically I avoided the Target card fiasco because I though it would add an opportunity for error but if I had the cards when she asked for payment I probably could have just read the numbers off.
And as I typed this I received a 2nd notification of payment text and my credit card has two $328 pending charges from AT&T... #facepalm
