Porting in from various networks: don't use Google Voice, Boost or any MVNO that uses Tmobile as their network. Even if the rep let's you do it, your rebate will get rejected later.
It is not known if you can stack the costco $200 with the tmobile $100 but one thing to note is that the costco rebate is by Wireless Associates which runs the booths in Costco, while the $100 is directly by T-mobile.

I ported a GV number and submitted the rebate on T-mobile's site. There have been mixed reports here so we'll see. (The rebate submission link is at the bottom of the T-mobile website. Use the 2020SwitchP2 promo and the number I ported in.) Costco asked what account number I was porting from and I told them just use the GV number as the account. That and the PIN is all I needed.
For the new line, they just need the IMEI of the BYOD phone. I didn't even need it with me, just the IMEI. They gave me a free SIM for it. They print out paperwork with all the info you need for the Costco rebate. I used the ported number for the submission.
Went to Costco at Fountain Valley, CA.
Manager there seemed to know that the free line and $200 rebate would apply for BYOD today. Brought my iphone 6s imei and got it setup in less than 30 min. Walked out w/ costco rebate instructions.
Free line notation still hasn't shown up on my account, but its only been 2 hours.
I ended up doing two different orders:
1) free line + 50% off Pixel + port-in
and
2) add new paid line + Costco $200 + port-in
I added this morning a new free line at Costco, ported in a Google Voice number with a spare unlocked phone I have which should qualify me for the Costco +$200 and the Tmobile +100 rebate.
I called the 611 and the representative confirmed me that I will qualify for the Google Pixel 4A promotions as I open a new line
My concern is that if I take the Google Pixel 4A promotion that might void or cancel my Costco BYOD $200 as I will submit it with the new GV ported number and IMEI of the spare phone. If I choose to get the Pixel 4A it will be a different IMEI at the newly added line (free) and the Costco BYOD +$200 gift card might not work.
Any ideas if I can still take advantages of the Google Pixel 4A promotion without sacrificing the Costco BYOD $200 gift card?
