The last "free line" wasn't free, and made me lose my grandfathered plan, even though I was promised multiple times it was fine. Proceed with caution.

Added line and did number porting at Costco. They did seem to know about the free line and said it would show up with a monthly fee but it'll be discounted on the account later. My account shows Line on Us 2 for $20, but that could be the previous promo since the current one seems to be Line on Us 3. I'm contacting T-mobile to make sure it's free.
I ported a GV number and submitted the rebate on T-mobile's site. There have been mixed reports here so we'll see. (The rebate submission link is at the bottom of the T-mobile website. Use the 2020SwitchP2 promo and the number I ported in.) Costco asked what account number I was porting from and I told them just use the GV number as the account. That and the PIN is all I needed.
For the new line, they just need the IMEI of the BYOD phone. I didn't even need it with me, just the IMEI. They gave me a free SIM for it. They print out paperwork with all the info you need for the Costco rebate. I used the ported number for the submission.

On Magenta2.

Went to Costco at Fountain Valley, CA.

Manager there seemed to know that the free line and $200 rebate would apply for BYOD today. Brought my iphone 6s imei and got it setup in less than 30 min. Walked out w/ costco rebate instructions.

Free line notation still hasn't shown up on my account, but its only been 2 hours.

Yep, that's what I did. I add two lines and my bill went up $25. We will see about that discount.

I didn't attempt to stack both of those because multiple people said they won't stack since Costco is BYOD and I didn't want to take the risk.

I ended up doing two different orders:

1) free line + 50% off Pixel + port-in

and

2) add new paid line + Costco $200 + port-in

I added this morning a new free line at Costco, ported in a Google Voice number with a spare unlocked phone I have which should qualify me for the Costco +$200 and the Tmobile +100 rebate.
I called the 611 and the representative confirmed me that I will qualify for the Google Pixel 4A promotions as I open a new line

My concern is that if I take the Google Pixel 4A promotion that might void or cancel my Costco BYOD $200 as I will submit it with the new GV ported number and IMEI of the spare phone. If I choose to get the Pixel 4A it will be a different IMEI at the newly added line (free) and the Costco BYOD +$200 gift card might not work.

Any ideas if I can still take advantages of the Google Pixel 4A promotion without sacrificing the Costco BYOD $200 gift card?

So it's legit? How long till the extra line shows up on your plan?
This was through the chat, I paid for the $10 sim card & $20 assisted service charge or something. Total 30.78 including tax. Couple of emails came through after paying, simcard will be mailed by 11/30 and then I just have to activate.

I have the same SOC number as you (SCFUTT4) and I was able to add a free line at Costco, BYOD with a ported number from Google Voice
I will give it a try. What I found that folks who has SoC SCUFTT4... they had changed some to SCUTTF4 (lol moved the F next to 4) to mark them in eligible. But will go to Costco and give it a shot. Thanks.

For costco rebate did you use the temporary number you got at costco or the ported number?
I submitted with the temporary number. Costco rebate requires the information that is on the receipt.

Will porting in from n10 work for the promo? It appears to be Verizon service but the account number is the serial number of the phone. I was wondering if anyone has done this successfully and what you're supposed to tell the T-mobile rep for the service?

Are you planning to take any Phone promotions?

I was told when I called 611 that I can also be eligible for the Google Pixel4A promotions. I'm just concern that might void my Costco BYOD Gift card $200

Sorry, I haven't seen a successful stacking of these two promos yet. You'd either lose the Pixel 4A promo or the Costco GC.

What is the SOC for your plan? This is the T-Mobile ONE plan with the ONE Plus promo add-on for each line correct? It's what I have, SOC FRLTUL4LP
I don't recall the SOC but I can check for you tonight (currently outside). But look at my screenshot as well ( I do have one plus promo like you mentioned) and all promos are working flawless.

According to the List of SOC

SCFUTT4 rate codes were moved to SCUTTF4 if they had a corporate discount specifically to make those accounts ineligible
