"Maintain rate plan and lines for 12 months to keep free line(s) . After the 12 months, cancel paid line without affecting free line(s)."
Anyone know for sure?
I believe you can switch the paid lines over to the free ones after a year, but you must maintain at least two paid lines.
Anyone else, capped at 4?
Not complaining, have 10 lines including taxes and fees for $80/mo. This would make my 11th.
When the time comes, Is there a more efficient way than porting out to another carrier and then porting back in?

First, I asked the rep to confirm that I was eligible for the free line deal. Rep pulled up my account and said yes (I had existing lines with T-Mobile One plan). I then asked what was the best current deal for trading in an iPhone XS. Two offers were presented:
Offer #1: "We have Buy one iPhone, get one on us! For a limited time in all markets, get up to $730 off when you buy two new Apple iPhone XR, iPhone XS, iPhone XS Max, iPhone 11, iPhone 11 Pro, iPhone 11 Pro Max, iPhone 12 mini, iPhone 12, iPhone 12 Pro, or iPhone 12 Pro Max phones (credit toward equal or lesser value device, via monthly Recurring Device Credits (RDC)) when you purchase both on EIP and active a new voice line on a qualifying rate plan (new customers must activate 2 new voice lines). This offer will appear on the customer's bill as "2020 Apple Buy 2 P11."
Offer #2: "Get an iPhone 12 Pro Max for $5/month. For a limited time in all markets, get up to $950 off a new Apple iPhone 12 Pro or iPhone 12 Pro Max (via monthly Recurring Device Credits RDC)*) when you purchase it on EIP, trade in a qualifying Apple iPhone, and activate a new line of qualifying voice service. This offer appears on the customer's bill as "2020 Apple Trade P27".
I specifically asked...multiple times...if I took advantage of these "add a line" offers, would it be a paid line or free line. Each time the rep confirmed that it would be free because I was eligible for the free line promo which could be combined with these offers. So I took offer #2, since it seemed to be the better deal and I only needed one phone.
When the order activity posted in my account, I could see that the new line was indeed free. When I received my first bill after the order was fulfilled, the additional line was free and I received all bill credits as described by the rep. Needless to say, I'm a very happy customer with T-Mo!

I haven't done any of these promos before and question the charges that I saw on the receipt but she assured me that the promotional credits would kick in on the first bill and I wouldn't be charged for the line or the phone.
Learned the hard way about three years ago when the agent assured me oh yeah you're qualified. Two months later I had to call and have them reverse all charges because they never gave me the credit and told me promo plans are never included.
