i did this earlier in the year on two lines and only got one rebate. rebate on other line just disappeared right about when i got the first rebate - it was showing the pending status up until that point and no invalid status.

i contacted the costco kiosk and they said i had to wait entire 14weeks which was last week. Only thing i can think is that i did a separate promo (iphone 12 deal from TMO) couple months after and on both lines. Only difference was on the line I didnt get the rebate, i did the promo through costco again but from everything I've seen, that shouldnt affect the rebate?

Depends on store, I have never had phones with me just give them imei and off I go. Also you can bring your own sims to avoid sim fee, Apple store gives them out for free. This was a Costco decision not WA to charge sim fee, perhaps complain enough and they may reconsider.

Does anyone ported from Tello? The wireless Kiosk at my Costco location said Tello is not qualified since it's a Sprint affiliate and TMobile & Sprint is now one company..

Yes but you need to use new devices because old devices got IMEI blocked by Tmo.
