For existing lines, at least, it appeared to be an X or higher. For new lines, I think it's any working phone.
Did you speak to a costco rep or verizon rep? I'm doing an upgrade as an existing customer. I have the unlimited plus plan (but have the 2nd gen SE that I want to trade in). I'd like to know if I'd be able to get $830 (sounds unlikely though)
There is no printout promising anything, however the trade in agreement that you sign does say that I will get the full $830 trade in credits provided my trade ins are in the condition that I reported, which was working with a cracked screen.
FWIW when I dropped the trade ins off at the Verizon store the rep didn't even turn them on and when I asked "don't you want to turn them on?" The rep said Verizon doesn't care when it's a damaged phone trade in. I find it hard to believe that's true, but I'm sure the damaged phone trade in promos are a method to get me to upgrade and locked in for 3yrs.

Speaking of which Metro offers roaming in Mexico and Canada for $5/mo which you can pay for only the month you are using it.
Of course you can call your friends and family with whatsapp anywhere anytime for free but for local calls like hotels and businesses its much easier to use a local sim/number, especially when you need to call 800, 888 and other free numbers which happens a lot with airlines, hotels, car rentals, credit card customer service, businesses etc so good luck using your Kentucky number while in Paris, roaming on your Verizon sim and trying to call the local 0800 car rental customer service.

Gives me some peace of mind that they won't come back and say it wasn't received in the correct condition.
Since opening with Verizon online get me free iPhone 14, I'll be opening 5 online and 3 with Costco (3 people want iPhone 15). Will I be getting $250 x 3 for opening 3 line and trade in ?
