Select "Other" for the carrier. I didn't see anything for Google.
The account number is your 10-digit Google Voice number.
For the address, use Google's address:

For the pin, you need to have a voicemail pin. If you don't have one:
Go to https://voice.google.co
Go to Settings -> Voicemail -> Call to listen
If this is unchecked, check it and it'll ask you to put in a pin.
If this is already checked, you should see a button to reset the pin if you forgot it.
A couple other notes I wanted to put out there:
There isn't any sort of branding when booting up the phone. I noticed that the A15 from the Straight Talk deal had a splash screen for the carrier when booting.
There wasn't anything branding related until I was going through the setup process. There was a setting for TracFone Wireless Device Pulse terms and conditions. I'm wondering if this screen still shows up once unlocked and is factory reset. Does anyone happen to know?
Overall, it was a pretty smooth process for me. Not bad for a $65 phone.

To unlock the Google Voice number for porting, it's $3. I couldn't find it on my phone at a glance, but you can also do it here, which is what I did: http://www.google.com/voice/unlock
If you're not seeing the phone for free, it might be because you didn't pick the right plan. It has to be the Total 5G+ Unlimited plan for $65, and not the $55 one. I tried the $55 plan at first too because it said it was eligible for the phone discount, but in the end, the phone wasn't free. So I switched to the $65 one, like the post originally said, and it was all good after that.
Total after taxes and fees was just about $85. The Chase offer will get me $15 back, and a CB site is currently 14% back, although I only got back $8.31. But in the end, it's about $62.
Not that it's a huge add-on value but the plan also has Disney+ with no ads.

Alternatively I also explore the option to unlock the bootloader and root devices for extended custom rom support (but that's not true for all devices. Having said that. Motorola is far better than Samsung on ease of bootloader unlocking and releasing the same hardware across the globe meaning better open source developer support.)
But that's just me, you may disagree and I respect that.

The problem I have with constantly upgrading phones yearly is that it is really unnecessary. It is more reasonable to do it every 2-3 years. If one can do 4 or more years, you can really feel and see the upgrade difference. Just because there are good deals, it doesn't mean to buy it. People may be really happy with their current daily driver phone but they will still buy another phone just because. With these mvno deals, they are solid but I don't want to go to the well too often and then possibly get my ID get blacklisted.
If one wants to "disconnect" from apps and try to stay off the phone, the iphone se 2022 is a good idea.

When I try to activate the phone on TotalVerizon.com it says i need to put in my existing carrier, but google voice is not listed. I can select Other, but then it asks for an account number. Anyone run into this issue and know what to do?

Well, the iphone SE is year a older now and things look different compared to what's available now. What I'm saying is phone deals in general were way better values during previous Black Friday sales.Slick Deals threads for some phone deals were hundred of pages long with thousands of posts. Not this year. The Walmart flyer used to be loaded with great deals on everything, TV's etc. I think this points to serious economic troubles coming in retail.
