-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

Now for some reason the Membership Wireless system would not let the sale proceed (it wasn't clear to me why) because the credit check was not showing up in their system, even though T-mobile had approved it. Then there was some more back and forth and finally the sale was approved.
Next I had to stand in the long line of turkey shoppers to actually buy the phone at one of the check stands. Then I had to pick up the phone from the Costco store room. Then wait for a "runner" as Costco will not let shoppers walk the phone back to the Wireless kiosk.
Finally it took almost another hour before the sale was completed. Again this involved 2 calls to T-mobile where the kiosk guy wanted Tmobile to create an account with a dummy IMEI so he could add it and the T-mobile guys refused. According to the kiosk guy the were 'trying to steal the sale' (something about how the sales reps get commissioned).
Eventually he gave up (and I think he let the T-mobile guys steal his commission). Then I had to sign in a zillion places, as if I was buying a car. He then printed out the RMA label for returning the trade in and gave me a nice plastic bag to carry all the paperwork and the phone.
The kiosk rep was a pro though and new all about the promo. My iphone 7 priced out at a low $68 and he said that was good, as that would be rebated as T-mobile credits whereas the remainder would get paid out as a rebate MC. He also did not waste any time on trying to upsell accessories and protection plans.

Also, these are first class mail, if you ship them together, it will be over 1 pound. So you might get charged at the post office or even get the package sent back to you for insufficient funds.

Anyway, she was able finally get it sorted and I walked out the door with my new phone a good 2.5 hours later. My advice:
- Call the store ahead of time (ideally right before you set out) to make sure they have the exact phone you want in stock.
- Try to go first thing in the morning or during off-peak times to avoid waiting (I had to wait 40 mins just to be seen).
- Triple-check that your trade-in phone is clear of all locks.
- Triple-check that the T-mo account holder is present (in my case it was my wife, who wasn't present, but our rep was cool about it because I'm added as an authorized user).
- The reps are super busy, and there are time gaps in the process e.g. when you go and purchase the phone in the checkout line. It doesn't hurt to remind them what exactly you're signing up for when you're back at the kiosk.
YMMV on the following things my rep told me without prompting:- Can use another line's SIM on the newly activated phone as long as it's a number on the same account.
- Only need to keep the new line active for 90 days, after which we can cancel (again, I didn't ask, she offered this info!).
