They'll fully gloss over the part where the case acts as a heatsink.
This is a really nice case, I have the older Pi3 version and love them. but the overall value of the Pi4 just isn't making any sense to me?

case: $12
power adapter: $10
32gb sdcard: $8
Pi4/4GB: $47 (usually $62)

$77 total for a minimal setup?

For about the same price, or less when on sale, I can get a HP thin client machine with a PCIe slot and so much more capability (upgradable RAM, SATA/M.2, etc)... the only thing the Pi4 setup has over a tiny thin client PC is that it's smaller.
It's also silent and only uses 4-5 watts. Plus there's a bunch of connectivity for more creative pursuits, but it sounds like you don't care about that.

The listing states that it comes with the thermal pad. You only need one.

I don't know if the 4 has a higher TDP than the 3 but this case works excellent with the 3. It gets warm but been stable for over a year 24/7.

It all depends on what you want to do. If you need GPIO, that's going to be easier with more support on the Raspberry Pi, and the Pi may have lower energy usage. In many situations, a thin client will be a better fit and as you said will have better upgradability. I haven't seen any thin clients though for under $100 new including RAM, PSU and storage. I would be very interested in that, any suggestions?

Damn I wish this could handle the POE hat. Looks like a great enclosure but I need POE.
