1) Going directly with T-mobile, using the iPhone 8 as a trade in will net you $415 off instead of Apple $140 + $150 Rebate Card. $415 vs $290. No brainer. Granted the Tmo deal is bill credits, but if your staying with Tmobile, who cares.
2) If you don't want Bill credits, BE PATIENT. Costco will have an offer soon if they don't have one already that will usually mirror T-Mobiles direct offer except with a prepaid card instead. Will beat Apple's crappy offer.

I got $265 for XR and 150 as gift card from T-mobile.
Con:
You need to have access to last 4 digits of your mom social
Once the new device is activated the sim on your mom's old phone will be deactivated.
If your mom is at a different location then she has to go to a T-mobile store to get the sim activated for her line.
The other option would be if you pickup the iphone 12 from apple store. You can get a T-Mobile sim free at Apple store.
