4) Follow voice prompt to enter IMEI, ZIP code, create PIN, etc. When asked by the prompt if you want to "add to an existing account", say "No". If you purchased a plan with the phone, it will recognize this and automatically activate the SIM for the plan.
When the voice prompt tells you to power on the device, the instructions were incomplete and unclear. If you've never activated a phone before here are the steps:
1) Attached to the box for your phone is the associated SIM card in a pouch; remove and punch out the nano-SIM.
2) Using the SIM tool in the iPhone box (or a small paperclip), pop out your SIM card tray and insert the nano-SIM.
3) Re-insert the SIM tray into the iPhone and power on the phone.
4) Go through the setup process; I recommend connecting to WiFi when prompted to make things go faster.
5) Your phone should be activated within a few minutes of being powered on with the SIM inserted, assuming you went through all the steps in the automated phone activation.
I'm not going to waste my time going there, but thought I'd help others. Be sure to call the local Walmart you are going to, ask if they have the phone in stock (both stores said online they didn't have stock, but both stores did have them in stock) and ask if they have the WARP system for activation. Went to Casino Walmart in Hammond, IN, cuz Cabelas Walmart in Hammond was confirmedly sold out. Had no problem there, other than the reps using the $45 and $55 plans to sign me up, instead of $35. I caught the first one, but they wouldn't add the $35 plan even when I asked about it. But I was over arguing at this point, and wanted 2 good deals rather than getting into a fight with a Walmart worker over it.
Saw in Stock online at Lansing, so I went there. They didn't have an employee who could activate/computer terminal to activate phones, but told me there were 7 in inventory at Casino WM. Called Cabelas WM, the guy told me he was selling his last ones. Was told by Cabela's Hammond that Casino had Activation Terminal/STMobile Employees. Was told by Casino ST Mobile Activation Lady they had none, but she would check. She found them, but said they weren't there last time she checked (maybe they got a shipment, maybe she wanted me to give up easier).

I plan to stay on Tello after the unlock. You pay for what you only need and do not play games with the customer.
