4) Follow voice prompt to enter IMEI, ZIP code, create PIN, etc. When asked by the prompt if you want to "add to an existing account", say "No". If you purchased a plan with the phone, it will recognize this and automatically activate the SIM for the plan.
When the voice prompt tells you to power on the device, the instructions were incomplete and unclear. If you've never activated a phone before here are the steps:
1) Attached to the box for your phone is the associated SIM card in a pouch; remove and punch out the nano-SIM.
2) Using the SIM tool in the iPhone box (or a small paperclip), pop out your SIM card tray and insert the nano-SIM.
3) Re-insert the SIM tray into the iPhone and power on the phone.
4) Go through the setup process; I recommend connecting to WiFi when prompted to make things go faster.
5) Your phone should be activated within a few minutes of being powered on with the SIM inserted, assuming you went through all the steps in the automated phone activation.
I'm not going to waste my time going there, but thought I'd help others. Be sure to call the local Walmart you are going to, ask if they have the phone in stock (both stores said online they didn't have stock, but both stores did have them in stock) and ask if they have the WARP system for activation. Good luck everyone.

Ended up getting two phones from the deal, just not the two with the $35.00 plans.

I did not ask to cancel, I did not get any email updates. I did however call on Friday and ask when my phones would be shipped and created two support tickets, one for each order. Its possible the resolution of support ticket was to cancel my orders.
I have a second Pending remote fulfillment set to be delivered today
Don't do a chargeback now. Do you still want the phones? Total is a sister company and the phones will work on their service YMMV.
i did it 3 days back..so
now i reported for all other orders..might take couple of more days
it will be big set back for ST
why are they still not processing the refunds..surprise..how could they keep the money for this long

Chargebacks are supposed to be used for fraud situations where you see a charge on your statement that is unrecognized and you never ever made. If you file a chargeback with the credit card company and then the phone is shipped out today or tomorrow, it throws a wrench in the process. Credit card company or straight talk company may think the customer is trying to scam for a free phone with a delay shipping.
People who are worried about future deals, why would you consider doing business with this company after they steal your money?
You can talk to a dozen reps and they will all tell you different things (one will say order is shipped, another will say order is cancelled). The reps have absolutely no clue what is going on.
