Come on. Do not add pre-conditions to earn more. We are talking about no pre-conditions to earn 2% cash back on everything purchase and you just can't find the card to earn more. If you like conditions such as annual fee, member to open a bank account etc, that is a different thing. People may not like those conditions.

Wait... $500 is considered a large medical expense? You must be living in Europe then. Try $5,500 for braces or $3000 to $5000 per dental implant with crown. This card isn't going to cut it. Go with the big boys like Chase Sapphire Reserve or Ink where you can get up to $1k in bonus points for spending 6k within 3 months. And yes I'm speaking from personal experiences. Put in the time and effort to take good care of your teeth early on and you will avoid these high expenses later on in life.

Open a Credit Karma free account and get a recent check from one of the big 3 credit reporting agencies. (You get one free one a year, unless you have fraud)
I'm not going to suggest that you contact the old debt holder asking for an offer in compromise to settle the debt because they don't care if it's settled. All that will do is reactivate the old debt. (this was my experience with my ex in trying to settle her medical bills)
Used to be a statute of 10 years, but that may depend on the state you reside in.

You can also dispute the old charge via the credit reporting agencies. Say, file a letter explaining that you were unemployed, had medical issues, etc. if applicable.

Wastewater didn't mail us a bill for 10-12 MONTHS, then they decide to take us to collections for $1200.
We've been caregivers for a blind friend of mine for 17 years (whom helped cover rent in exchange for her care) and she recently lost a leg to gangrene.
So she's been in a nursing home for the last 2 years and unable to help with bills.
Once that went to collections we haven't been able to catch up with it.
And both my girlfriend and I have checkered credit.
We live in Kansas

I hear ya. It isn't easy but getting a decent credit history can be done.
What I discovered on my path to trying to get things "right" is that in most cases, the actual debt is bought by a collections agency for pennies on the dollar, and that the businesses simply write off the debt.
In your case, if the debt is still being held by the original owner, maybe an offer in compromise could be in order? Or at least offer payments to get to $1200, as long as they agree to mark the account paid in full.
If you sign up for CK the debt should be listed and whom it is held by. They also have links that will suggest credit cards / offers that you could qualify for.
To rebuild your credit, and get a credit card, look for banks that will let you establish some type of escrow (deposit money in the bank) and issue you a card that will be limited by the escrow. Or, get a high risk card? Max it out and pay it off every month, and you will rebuild your credit one month at a time. (took my sister 4 years but she is in the 800's now)

You really have to be honest with yourself though. The above path is only if you can be responsible with credit and pay any balances in full each and every time. Interest rates are at outlandish rates and you will dig yourself in a hole quickly if you try to make payments.

Be on the lookout in mail or online for 0% APR card for 12-18 months (if possible). Negotiate or have someone negotiate the bill down to something more affordable and say you'll cover it all at once for $500 or something (and place on 0% APR card if you can find one).

Now, if you NEVER travel.... well, I'd still take $200 plus 5% back on groceries all year before this one... and it'd give you more reason to obtain BoA status to get their 2.625% on everything option too...
How is Amex MR worth more? I have Amex and would like to know. To make use of them you need to book via their portal, which has higher costs, and much worse experience than booking via airline or via google flights etc. Same goes for Chase or any other portal.

And how much more are we talking? This is plain and simple cash back, not MR that you need to redeem. I got my Amex because of its PriorityPass benefit which makes it worth the $100 fee but I'm thinking of canceling it.

By transferring to travel partners for the most part- though there's potentially other ways to make them worth more depending on your other Amex cards and accounts.

For example there a card that will get you a 35% point refund for airline bookings in the portal (one airline of your choice in coach, any airline you want in business class), effectively making all points 1.35 cents instead of 1....so a 2x earning card would be 2.7% cash equivalent.

The PP benefit on Amex is actually the worst in the industry since it does not give you free meals at restaurants like the benefit from Chase or Citi does.

There's many OTHER reasons one might want to hold amex cards, or learn how to get value from their points- but that's not really one of em.
